LICENSED PHYSICAL THERAPIST 
Direct Hire

Job Summary
We are seeking a compassionate and motivated Licensed Physical Therapist to join our growing home health team. This role provides one-on-one patient care in a home setting, focusing on improving mobility, function, and overall quality of life. New graduates are encouraged to apply—we are willing to train the right candidate! This position offers a flexible schedule, competitive salary, and a supportive team environment with no nights, weekends, or holidays required.

Service Area
Shiner, Moulton, Hallettsville, Flatonia, Schulenburg, La Grange, Weimar, Columbus, and surrounding areas

Compensation & Schedule
  • Salary: $50 -$55 per hour 
  • Schedule: 32 hours/week (opportunity to work more) 
  • Mileage Reimbursement: $0.625 per mile 
  • Sign-On Bonus: $4,000 total 
    • $500 at hire 
    • $1,000 at 3 months 
    • $1,000 at 6 months 
    • $1,500 at 12 months 
  • Benefits: 
    • No nights, weekends, or holidays 
    • Paid major holidays 
    • Paid Time Off (PTO)
 
Essential Job Functions
  • Provide direct patient care in a home health setting 
  • Evaluate patient needs and develop individualized treatment plans 
  • Implement and adjust care plans based on patient progress 
  • Collaborate with and supervise Physical Therapy Assistants 
  • Educate patients and families on treatment plans and home exercise programs 
  • Maintain accurate and timely documentation of patient care 
  • Communicate effectively with interdisciplinary care teams 
  • Ensure compliance with safety, infection control, and quality standards 
  • Participate in ongoing professional development 
  • No OASIS documentation required
Job Requirements
  • Graduate of an APTA-accredited Physical Therapy program 
  • Current Texas Physical Therapist license 
  • New graduates welcome (training provided) 
  • Home health or geriatric experience preferred 
  • Ability to lift, transfer, and assist patients as needed 
  • Strong communication, organization, and documentation skills 
  • Ability to work independently and manage time effectively
